If you are seeking to have commissions paid to a licensed entity or agency, you should complete the below entity agreement. Please make sure that you have E&O coverage for both the agency and the signing officer. In addition, the signing officer must be licensed. When doing business in different states you must have both the entity and the signing officer licensed in the situs state of the group/client.
